The Central Remedial Clinic is seeking a dedicated Community Connections Officer to join its Adult Day Services team in Clontarf, Dublin. Job Overview

As a Community Connections Officer, you will play a hands-on role in empowering individuals to access education, employment, and social opportunities. Working within a human rights-based framework and aligned with HSE New Directions standards, you will collaborate with service users, families, and multidisciplinary teams to design personalised pathways that promote independence and inclusion.

Key Responsibilities

  • Support adults with complex needs to engage in community-based activities and opportunities
  • Co-design personalised development plans with service users and their support networks
  • Facilitate access to education, training, employment, and social inclusion initiatives
  • Work collaboratively with multidisciplinary teams to deliver person-centred supports
  • Promote independence, choice, and dignity in all aspects of service delivery
  • Ensure services align with HSE New Directions and Interim Standards
  • Maintain accurate records and contribute to service planning and evaluation
  • Adapt working hours as required, including occasional evenings and weekends

Required Skills & Qualifications

  • Relevant qualification in Social Care or a related discipline
  • Experience supporting individuals with complex needs in a community setting
  • Strong interpersonal and communication skills
  • Ability to work collaboratively within multidisciplinary teams
  • Knowledge of person-centred planning and human rights-based approaches
  • Flexibility to work across multiple Dublin locations
  • Full driving licence may be advantageous
