The Central Remedial Clinic is seeking a Community Access Facilitator to join its Adult Day Services team in Clontarf, Dublin. Job Overview

As a Community Access Facilitator, you will work directly with individuals, their families, and multidisciplinary teams to co-design personalised pathways for social participation, education, and independent living. This role is grounded in a Human Rights Based Approach and supports the delivery of high-quality, community-focused services.

Key Responsibilities

  • Support adults with complex needs to engage in meaningful community activities
  • Collaborate with individuals and their support networks to design personalised development plans
  • Facilitate access to education, employment, and social opportunities
  • Work closely with multidisciplinary teams to ensure holistic care delivery
  • Promote independence, inclusion, and active citizenship
  • Implement practices aligned with HSE New Directions and Interim Standards
  • Maintain accurate records and contribute to service improvement initiatives

Required Skills & Qualifications

  • Relevant qualification in social care, healthcare, or a related field
  • Experience supporting individuals with complex needs is highly desirable
  • Strong interpersonal and communication skills
  • Ability to work collaboratively within multidisciplinary teams
  • Commitment to person-centred and rights-based approaches
  • Flexibility to work across multiple locations and varied hours
