The Central Remedial Clinic is seeking a Community Access Facilitator to join its Adult Day Services team in Clontarf, Dublin. Job Overview
As a Community Access Facilitator, you will work directly with individuals, their families, and multidisciplinary teams to co-design personalised pathways for social participation, education, and independent living. This role is grounded in a Human Rights Based Approach and supports the delivery of high-quality, community-focused services.
Key Responsibilities
- Support adults with complex needs to engage in meaningful community activities
- Collaborate with individuals and their support networks to design personalised development plans
- Facilitate access to education, employment, and social opportunities
- Work closely with multidisciplinary teams to ensure holistic care delivery
- Promote independence, inclusion, and active citizenship
- Implement practices aligned with HSE New Directions and Interim Standards
- Maintain accurate records and contribute to service improvement initiatives
Required Skills & Qualifications
- Relevant qualification in social care, healthcare, or a related field
- Experience supporting individuals with complex needs is highly desirable
- Strong interpersonal and communication skills
- Ability to work collaboratively within multidisciplinary teams
- Commitment to person-centred and rights-based approaches
- Flexibility to work across multiple locations and varied hours
