Job Overview
As a Community Support Worker with Central Remedial Clinic, you will work directly with individuals to support their personal goals through a Person-Centred Planning (PCP) approach. This role combines hands-on care with community-based engagement, helping individuals live self-directed and fulfilling lives.
You will be part of a dynamic and flexible service model, with working hours aligned to individual needs across a Monday to Sunday roster between 8am and 8pm.
Key Responsibilities
- Support individuals in achieving personal goals through Person-Centred Planning
- Provide high-quality personal care tailored to individual needs
- Facilitate community participation and social inclusion activities
- Assist with daily living tasks and promote independence
- Work collaboratively with multidisciplinary teams and families
- Maintain accurate records and contribute to care planning reviews
- Adapt to flexible working hours including evenings and weekends
Required Skills & Qualifications
- Relevant qualification in healthcare, social care, or related field
- Experience supporting individuals with complex needs (desirable)
- Strong interpersonal and communication skills
- Commitment to person-centred care and inclusion
- Ability to work flexibly across a 7-day roster
- Understanding of HSE New Directions framework (desirable)

This is a Fixed Term Wholetime position with working hours scheduled between 8:00 AM and 8:00 PM, Monday to Sunday. Flexibility is essential as services are tailored to the needs of individuals.
