Galway University Hospitals are seeking an experienced Clinical Nurse Specialist (Integrated Care – Diabetes) to support high-quality, patient-centred diabetes care across hospital and community settings. This role plays a key part in integrated care pathways, education, and multidisciplinary collaboration to improve outcomes for people living with diabetes.
This is a specified purpose, wholetime position within a major teaching hospital group in the HSE West and North West region.
Key Responsibilities
• Deliver specialist nursing care for patients with diabetes across acute and community settings
• Lead and support integrated care pathways in collaboration with multidisciplinary teams
• Provide expert clinical advice, education, and support to patients, families, and staff
• Promote self-management, prevention, and early intervention strategies
• Participate in service development, audit, and quality improvement initiatives
• Contribute to education and training programmes for nursing and healthcare staff
• Maintain accurate clinical documentation and adhere to professional standards
Required Skills & Qualifications
• Registration as a Nurse on the live Register of Nurses and Midwives maintained by NMBI
• Post-registration qualification relevant to diabetes care (essential)
• Minimum required post-registration clinical experience appropriate to CNS grade
• Demonstrated specialist knowledge and clinical competence in diabetes management
• Excellent communication, leadership, and organisational skills
• Experience working within multidisciplinary and integrated care models
Salary, Benefits & Working Hours
• Grade: Clinical Nurse Specialist (General) – 2632
• Salary in line with HSE Consolidated Salary Scales (effective 1 August 2025)
• Specified Purpose, Wholetime contract
