The Health Service Executive (HSE) is seeking a dedicated Clinical Nurse Specialist (Cardiovascular Disease) to join the Integrated Care Hub in Castlebar, Co. Mayo. This permanent, whole-time position supports integrated care for patients across Galway, Mayo, and Roscommon.
About the Role
As a Clinical Nurse Specialist in Cardiovascular Disease (CVD) – Integrated Care, you will deliver expert clinical nursing and education to enhance prevention, management, and recovery outcomes for patients with chronic cardiovascular conditions. You’ll collaborate closely with multidisciplinary teams to ensure person-centred, evidence-based care across hospital and community settings.
Key Responsibilities
- Provide specialist clinical assessment and management for patients with cardiovascular disease.
- Develop, implement, and evaluate care pathways within the integrated care framework.
- Lead health promotion and secondary prevention initiatives in cardiac health.
- Support patient self-management and adherence to treatment plans.
- Deliver education and mentorship to nursing and allied health staff.
- Collaborate with GPs, hospital specialists, and community care teams.
- Participate in research, audit, and continuous quality improvement.
Required Qualifications
Essential
- Registered Nurse (General) with the Nursing and Midwifery Board of Ireland (NMBI).
- Significant clinical experience in Cardiology or Integrated Care.
- Evidence of ongoing professional development relevant to the specialty.
Preferred
- Postgraduate qualification in Cardiology, Chronic Disease, or Integrated Care.
- Experience working within the HSE Integrated Care Programme for Chronic Disease.
