Join the Health Service Executive (HSE) as a Senior Medical Social Worker (Grade Code 3019) supporting older persons in an in-patient hospital environment. About the Role
This specified-purpose, wholetime position is based in Galway, under the HSE West and North West region. The successful candidate will bring depth and breadth of experience working with Older Persons in in-patient settings, focusing on seamless hospital discharge and coordinated care.
Key Responsibilities
- Lead complex discharge planning for older persons.
- Collaborate effectively within a multidisciplinary healthcare team.
- Provide psychosocial assessment and intervention to patients and families.
- Support hospital staff through expert consultation and case discussions.
- Contribute to service development and best practice initiatives.
- Maintain accurate records and uphold HSE policies.
- Engage in supervision and continuous professional development.
Required Qualifications
Essential
- Recognised qualification in Social Work.
- CORU registration as a Social Worker.
- Demonstrated experience working with Older Persons in an acute or in-patient setting.
- Proven record of multidisciplinary teamwork and collaboration.
Preferred
- Experience in discharge coordination or hospital-based care.
- Postgraduate training relevant to gerontology or medical social work.
