The Health Service Executive (HSE) West and North West is inviting applications for the position of Senior Medical Social Worker (Grade 3019). This specified-purpose wholetime role is based across Sligo and South Donegal, offering the opportunity to work as part of an integrated multidisciplinary team in acute, primary care, and community settings.
The successful candidate will deliver a comprehensive social work service to patients and their families, focusing on assessment, support, advocacy, and coordination of care. The post will particularly suit candidates with experience in dementia care and supporting older people and their carers.
This position is open to both HSE and non-HSE applicants.
Key Responsibilities
- Provide professional social work services across acute, primary, and mental health settings.
- Deliver person-centred care planning for older adults and individuals living with dementia.
- Collaborate with multidisciplinary teams to support discharge planning and integrated care pathways.
- Promote advocacy, empowerment, and the protection of vulnerable individuals.
- Maintain high standards of professional practice and ethical conduct in line with CORU registration requirements.
- Contribute to service evaluation and quality improvement initiatives.
Essential Requirements
- Professional qualification in Social Work (National Qualification in Social Work (NQSW) or equivalent recognised by CORU).
- CORU registration as a Social Worker.
- Demonstrable experience in medical social work across multiple care settings.
- Evidence of experience supporting individuals with dementia and their carers.
- Excellent communication, assessment, and problem-solving skills.
- Full clean driving licence and access to own transport (as service delivery may span multiple sites).
Desirable Attributes
- Postgraduate qualification in Gerontology, Dementia Care, or related field.
- Experience with integrated care for older persons or community-based social care programmes.
