Cork University Hospital is seeking an experienced Clinical Nurse Manager 2 to lead Discharge Coordination services on a 12-month specified-purpose contract. This role offers an excellent opportunity to contribute to patient flow, multidisciplinary communication, and service efficiency within Ireland’s largest university hospital.
As part of the Health Service Executive, you will play a central role in coordinating complex discharges and supporting safe transitions of care.
Key Responsibilities
• Lead and manage the Discharge Coordination service at Cork University Hospital
• Oversee timely, safe discharge planning with multidisciplinary teams
• Support patients and families in navigating post-acute services
• Coordinate with community and regional services to ensure continuity of care
• Monitor discharge processes, identify delays, and implement service improvements
• Provide clinical leadership, guidance and mentoring to nursing staff
• Maintain accurate records and support audit, reporting and compliance
• Contribute to hospital-wide patient flow initiatives
Required Skills & Qualifications
• Registered General Nurse (RGN) with active NMBI registration
• Minimum of 5 years post-registration nursing experience
• Proven leadership or supervisory experience at CNM1/CNM2 level desirable
• Strong knowledge of discharge planning and community care pathways
• Excellent communication, coordination and problem-solving skills
• Ability to work effectively in multidisciplinary and fast-paced environments
• Commitment to quality improvement and patient-centred care
Salary, Benefits & Working Hours
Salary: CNM2 (Grade 2119) — €61,463 to €77,666 per annum, per the HSE Consolidated Salary Scales effective 1 August 2025
Contract type: Specified Purpose, Wholetime
Working hours: Full-time
