Join the Health Service Executive as a Clinical Nurse Specialist in Cardiovascular Disease Integrated Care, serving the Cavan and Monaghan region. This permanent, whole-time post offers the opportunity to deliver specialist cardiovascular management and prevention services within a dynamic community and acute care pathway. The role supports integrated care delivery across HSE Dublin and North East.

Key Responsibilities
• Deliver specialist cardiovascular nursing expertise across community and acute settings
• Support assessment, diagnosis, treatment, and monitoring of patients with cardiovascular disease
• Coordinate integrated care pathways to improve patient outcomes
• Provide education, self-management support, and risk-factor modification guidance
• Collaborate with GPs, acute hospitals, multidisciplinary teams, and integrated care programmes
• Contribute to audit, quality improvement, service development, and clinical governance
• Maintain accurate documentation and comply with HSE standards and policies
Required Skills & Qualifications
• Registered General Nurse with active NMBI registration
• Minimum of 5 years post-registration experience, including substantial cardiovascular or relevant specialist practice
• Postgraduate qualification in cardiovascular nursing or commitment to complete one
• Demonstrated ability to work autonomously within integrated care models
• Excellent communication, clinical decision-making, and patient-education skills
• Proven experience in multidisciplinary collaboration
• Commitment to ongoing professional development
Salary, Benefits & Working Hours
Grade: Clinical Nurse Specialist (General) – 2632
Salary Scale (1 August 2025): €61,463–€77,666 (LSI) • based on HSE consolidated salary scales
Contract Type: Permanent, Wholetime
Working Hours: Full-time, aligned with HSE nursing and midwifery rosters
