Key Responsibilities
- Provide expert clinical pharmacy services to hospice and palliative care patients.
- Support safe, effective, and evidence-based medication management practices.
- Collaborate with multidisciplinary teams to optimise therapeutic outcomes.
- Oversee medication governance, audit, and compliance processes.
- Deliver clinical advice and education to healthcare professionals.
- Participate in service development and quality improvement initiatives.
- Ensure adherence to regulatory and professional standards.
Required Skills & Qualifications
- Registered Pharmacist with the Pharmaceutical Society of Ireland (PSI).
- Significant post-registration experience, ideally in hospital or specialist care settings.
- Demonstrated experience in clinical pharmacy practice.
- Strong knowledge of medication safety, governance, and regulatory compliance.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to patient-centred care in a hospice or palliative setting.
