Job Overview
This Clinical Nurse Manager 2 Discharge Coordinator position is a permanent, full-time role within Connolly Hospital, part of the HSE Dublin and North East region. Key Responsibilities
- Lead and coordinate effective patient discharge planning across all relevant departments
- Collaborate with multidisciplinary teams to ensure timely and safe patient transitions
- Monitor patient flow and implement strategies to reduce delays in discharge
- Support clinical staff in discharge planning best practices and protocols
- Ensure compliance with HSE policies and national healthcare standards
- Maintain accurate documentation and reporting related to discharge processes
- Engage with patients and families to support informed discharge decisions
- Contribute to service improvement initiatives within unscheduled care
Required Skills & Qualifications
- Registered General Nurse with active registration on the NMBI register
- Minimum of 5 years post-registration clinical experience
- Proven leadership experience at Clinical Nurse Manager level or equivalent
- Strong knowledge of discharge planning and patient flow management
- Excellent communication, coordination, and organisational skills
- Ability to work effectively within multidisciplinary teams
- Demonstrated commitment to quality improvement and patient safety
- Relevant postgraduate qualification desirable
Salary, Benefits & Working Hours
Salary scale for this Clinical Nurse Manager 2 role ranges from €62,078 to €78,443, in line with HSE pay structures, inclusive of Long Service Increments (LSI).
