Corlann Disability Services is seeking a compassionate and motivated Community Facilitator to support individuals accessing community-based services across Roscommon. Based across the Strokestown, Elphin, Tulsk, Ballinagare, and Ballaghaderreen areas, this Community Facilitator role is ideal for candidates passionate about disability support, community engagement, and person-centred care within the HSE West and North West region.
Job Overview
The Community Facilitator will work closely with individuals using Corlann Disability Services to support access to social, educational, recreational, and community opportunities. The role focuses on encouraging independence, building confidence, and supporting service users to achieve personal goals within their local communities.
This position is permanent, part-time, and pensionable, offering long-term career stability within a respected disability support organisation operating under the HSE Section 38 framework.
Key Responsibilities
- Support individuals with disabilities to access community activities and services
- Promote person-centred planning and independent living skills
- Assist service users in achieving social, educational, and personal development goals
- Develop positive working relationships with families, colleagues, and community organisations
- Encourage participation in recreational and community inclusion programmes
- Maintain accurate records and documentation in line with organisational policies
- Provide practical and emotional support in a respectful and empowering manner
- Contribute to a safe, supportive, and inclusive service environment
Required Skills & Qualifications
- Experience supporting individuals with disabilities or community care services is desirable
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Commitment to person-centred approaches and social inclusion
- Flexibility to work across multiple locations within County Roscommon
- Understanding of safeguarding and healthcare support practices
- A full driving licence may be advantageous for community-based duties
