Community Healthcare Sligo Leitrim is recruiting Health Care Assistant Home Support staff to deliver practical, person-centred care to people living in their own homes across Sligo and Leitrim. Formerly known as the Home Help service, these roles support service users to maintain their independence, dignity and quality of life within their communities.

Role Overview
The Health Care Assistant Home Support role is part of the HSE Home Support Service and involves delivering care in accordance with each service user’s assessed needs and individual care plan. Successful candidates will work under the direction of the Home Support Resource Manager or another designated healthcare professional.
The service supports people to remain safely and independently in their own homes for as long as possible. Appointments may be offered on permanent or specified-purpose contracts and on a full-time or part-time basis.
Key Responsibilities
- Provide safe, respectful and person-centred personal care based on each service user’s care plan.
- Assist with activities of daily living, including personal hygiene, dressing, mobility, continence care and mealtime support where required.
- Encourage service users to retain their independence, abilities, choices and involvement in everyday tasks.
- Provide appropriate food preparation or domestic support where this forms part of an assessed care need.
- Observe and promptly report changes or concerns relating to a service user’s health, circumstances or wellbeing.
- Maintain accurate schedules, records, timesheets and other required documentation.
- Report accidents, incidents, complaints and safeguarding concerns through the appropriate HSE procedures.
- Work collaboratively with service users, families, carers, Home Support management and other healthcare professionals.
- Follow HSE requirements relating to confidentiality, infection prevention, manual handling, health and safety and risk management.
Requirements and Qualifications
- Candidates must satisfy the professional qualification and eligibility requirements for Health Care Assistant (Home Support), grade code 6494, as detailed in the official campaign documentation.
- A full driving licence is required.
- Access to suitable transport is required in order to fulfil the duties of the post across Sligo and Leitrim.
- An understanding of person-centred care, dignity, privacy, confidentiality and safeguarding is essential.
- Candidates should be able to work independently in service users’ homes while remaining part of a wider multidisciplinary service.
- Good communication, organisation and practical problem-solving skills are required.
- Flexibility to meet the operational requirements of the Home Support Service may be necessary.
Salary
€36,651 – €45,939 per annum on the nine-point HSE Health Care Assistant (Home Support) grade 6494 scale. This salary is based on the HSE Consolidated Salary Scales effective 1 June 2026.
Salaries for part-time appointments will be calculated pro rata according to the contracted hours worked. New appointees will normally commence at the appropriate entry point, subject to HSE incremental credit rules.
Location
Posts are available across Sligo and Leitrim. The work is community-based and will involve travelling between service users’ homes to provide care and support in accordance with assessed needs and scheduled visits.
