About the Role
As a Care Coordinator, you’ll play a vital role in ensuring that our clients receive timely and effective care support in their homes. You’ll manage the coordination of care plans, scheduling, and communication between clients, families, and our team of Carers.
You’ll be a key link in our service delivery — combining empathy, efficiency, and excellent communication skills to ensure smooth operations and exceptional care.
Key Responsibilities
- Coordinate home support services in collaboration with the HSE and Irish HomeCare management.
- Manage client referrals, ensuring accurate assessment and matching with the most suitable Carers.
- Create and maintain effective Carer rosters and client schedules.
- Provide ongoing communication and support to clients, families, and staff.
- Monitor service quality, promptly resolving scheduling or care issues.
- Liaise with multidisciplinary teams to ensure compliance with care plans and standards.
- Contribute to continuous service improvement and operational efficiency.
Required Qualifications
Essential
- Experience in care coordination, administration, or customer service.
- Strong IT literacy and confidence using scheduling systems or CRM tools.
- Excellent organisational, time management, and communication skills.
- A compassionate, respectful approach to clients and colleagues.
- Ability to remain calm and proactive in a fast-paced environment.
Preferred
- A Health or Social Care qualification (QQI Level 5 or above).
- Prior experience in home care, nursing, or community health settings.
