HSE West and North West is seeking a dedicated Case Coordinator to join our Physical & Sensory Disabilities Services team in Sligo. About the Role
We’re looking for an experienced Case Coordinator to support people with physical and sensory disabilities who wish to live independently in the community. Based in Sligo, you’ll work within our specialized Disability Services team covering Sligo and Leitrim, providing essential coordination and support services that enable individuals to achieve their personal goals and maintain community integration.
Key Responsibilities
- Coordinate comprehensive case management services for individuals with physical and sensory disabilities
- Develop and implement person-centered support plans in collaboration with service users and multidisciplinary teams
- Conduct assessments and reviews to identify individual needs and appropriate interventions
- Liaise with external agencies, families, and community organizations to ensure coordinated service delivery
- Maintain accurate case records and documentation in compliance with HSE standards and data protection requirements
- Advocate for service users’ rights and facilitate access to appropriate community supports and resources
- Monitor service quality and outcomes, providing regular reports and recommendations for service improvements
Required Qualifications
Essential Requirements
- Third level qualification in Social Work, Social Care, Psychology, or related disability studies field
- Minimum 2-3 years experience working within disability services or related healthcare environment
- Demonstrated experience supporting people with physical and sensory disabilities in community settings
- Strong understanding of disability legislation, rights-based approaches, and person-centered planning
- Excellent communication, assessment, and case management skills
Preferred Qualifications
- Postgraduate qualification in disability studies or related field
- Experience with HSE systems and procedures
- Knowledge of assistive technology and accessibility solutions
- Irish language skills (Gaeilge) would be beneficial given the bilingual nature of the role
- Valid driving license and access to transport for community-based work
