About the Role
The HSE Dublin and Midlands Region is seeking an experienced Community Healthcare Network Manager to lead and coordinate service delivery within Community Healthcare Network 12 (South Laois). This post represents a strategic leadership role within the Primary Care Services, responsible for ensuring the integration of community health and social care services across multidisciplinary teams.
The successful candidate will play a pivotal role in implementing HSE’s national Enhanced Community Care (ECC) programme and advancing patient-centred care in the region.
This is a permanent, full-time position, offering a rewarding opportunity to make a direct impact on health outcomes across the Dublin and Midlands region.
Key Responsibilities
- Provide strategic and operational leadership across Community Healthcare Network 12.
- Oversee service delivery for Primary Care, Social Care, and Mental Health within the assigned network.
- Manage and support multidisciplinary teams, ensuring integrated and coordinated care pathways.
- Develop and implement operational plans in line with HSE policy, performance targets, and budgetary frameworks.
- Foster stakeholder engagement with service users, GPs, community partners, and voluntary organisations.
- Monitor key performance indicators and support quality improvement initiatives.
- Report to the General Manager, Primary Care Services, on performance outcomes and resource management.
Required Qualifications
Essential
- Hold a relevant qualification at Level 8 NFQ or higher in health, social care, management, or a related discipline.
- Have significant management experience (minimum 3 years) within a healthcare or social care setting.
- Demonstrate knowledge of HSE structures, Primary Care, and Community Healthcare Networks.
- Strong leadership, analytical, and communication skills.
- Proven record in service improvement, staff management, and budget oversight.
Desirable
- Postgraduate qualification in Healthcare Management, Public Administration, or equivalent.
- Experience implementing integrated care or enhanced community care models.
- Familiarity with performance management systems and health data reporting.
