The Advanced Nurse Practitioner Candidate (General) role within the Integrated Care Programme for Older Persons (ICPOP) offers an excellent opportunity to develop advanced clinical expertise while delivering high-quality, person-centred care. Based in Tralee, this position focuses on improving outcomes for older adults through integrated and community-based healthcare services.
Key Responsibilities
- Provide advanced nursing care to older persons within the ICPOP framework
- Support integrated care delivery across community and acute services
- Work collaboratively with multidisciplinary teams to optimise patient outcomes
- Participate in assessment, diagnosis, and management of patient care needs
- Contribute to service development and quality improvement initiatives
- Maintain accurate clinical documentation and reporting standards
- Engage in continuous professional development and training
Required Skills & Qualifications
- Registered Nurse with NMBI registration
- Eligibility to progress to Advanced Nurse Practitioner level
- Relevant postgraduate qualifications (or working towards same)
- Experience in older persons care or community healthcare
- Strong clinical assessment and decision-making skills
- Excellent communication and teamwork abilities
- Commitment to evidence-based practice and patient-centred care
Salary, Benefits & Working Hours
Salary Scale: €71,432 – €80,671 per annum (aligned with HSE pay scales)
Contract Type: Specified Purpose, Wholetime
