The Health Service Executive (HSE) West and North West is Ireland’s national health service, providing comprehensive healthcare services across Donegal, Sligo, Leitrim, Roscommon, Mayo, and Galway. We are seeking an experienced Clinical Nurse Manager 3 to join our Older Persons Services team as an Integrated Discharge Manager, working across Community Healthcare West and acute hospital settings to enhance patient care transitions.
About the Role
This is a permanent wholetime Clinical Nurse Manager 3 position focused on integrated discharge management for older persons services across multiple healthcare settings. You’ll work flexibly between Community Healthcare West community-based services and hospital services including Galway University Hospital, Portiuncula Hospital, and Mayo University Hospital, with your base at Merlin Park Hospital, Galway. This senior nursing management role requires expertise in discharge planning, care coordination, and managing complex transitions for older patients across the care continuum.
Key Responsibilities
- Lead and manage integrated discharge processes for older persons across acute and community healthcare settings
- Coordinate care transitions between hospital and community services to ensure seamless patient journeys
- Develop and implement discharge planning protocols and best practice guidelines for older persons services
- Collaborate with multidisciplinary teams across Galway University Hospital, Portiuncula Hospital, and Mayo University Hospital
- Manage and supervise nursing staff involved in discharge planning and care coordination activities
- Ensure compliance with HSE policies, procedures, and quality standards for discharge management
- Monitor and evaluate discharge outcomes to drive continuous improvement in patient care transitions
Required Qualifications
Essential Requirements
- Bachelor’s degree in Nursing or equivalent professional qualification
- Current registration with the Nursing and Midwifery Board of Ireland (NMBI)
- Minimum 7-10 years post-registration nursing experience with substantial experience in discharge planning or older persons care
- Previous nursing management or leadership experience in acute or community healthcare settings
- Demonstrated expertise in care coordination and multidisciplinary team working
Preferred Qualifications
- Postgraduate qualification in nursing management, gerontology, or related healthcare field
- Experience working across both acute hospital and community healthcare environments
- Knowledge of HSE integrated care pathways and discharge processes
- Strong communication and stakeholder management skills with experience in change management
