Autism Initiatives Ireland is seeking a dedicated Facilitator Disability in Celbridge to provide person-centred support to individuals with complex needs. Key Responsibilities
- Provide person-centred support to individuals with complex needs.
- Implement and contribute to Individual Support Plans and Person-Centred Plans.
- Act in accordance with the Key Worker/Support Worker philosophy.
- Support service users in achieving personal goals and developing life skills.
- Promote independence, dignity, and community participation.
- Maintain accurate records and documentation in line with organisational policies.
- Work collaboratively as part of a multidisciplinary team.
- Adhere to safeguarding, health and safety, and regulatory standards.
Required Skills & Qualifications
- Full QQI Level 5 Health Care qualification (Health, Community, Care sector or equivalent) – essential.
- Genuine passion for providing person-centred support.
- Ability to work on own initiative.
- Understanding of the Key Worker/Support Worker role.
- Experience in programme planning and developing Person-Centred Plans.
- Fluent English with excellent communication skills.
- Full, clean manual driving licence – essential.
