Autism Initiatives Ireland is seeking a dedicated Facilitator Disability (Community Access Facilitator) to join their team in Wicklow Town. Key Responsibilities
- Provide person-centred support tailored to individual needs and goals
- Assist individuals in accessing community activities and services
- Develop and implement Person Centred Plans and support programmes
- Promote independence, dignity, and inclusion at all times
- Work collaboratively with multidisciplinary teams and families
- Maintain accurate records and reports in line with organisational policies
- Support individuals with daily living activities where required
Required Skills & Qualifications
- QQI Level 5 qualification in Health Care, Community Care, or a related field (essential)
- Strong commitment to person-centred care and support
- Ability to work independently and use own initiative
- Understanding of Key Worker/Support Worker roles and best practices
- Experience in programme planning and delivering person-centred plans
- Fluent English with excellent communication and interpersonal skills
- Full, clean driver’s licence (desirable)
Salary, Benefits & Working Hours
Salary Scale: €50,945 – €53,614 (pro rata)
Working Hours: 30 hours per week (Permanent Part-time)
