Job Overview
The Facilitator Disability Kildare position focuses on delivering high-quality, person-centred support within a structured service environment. The successful candidate will support individuals with complex needs, assist with programme planning, and promote independence and inclusion.
This is an excellent opportunity for someone with a QQI Level 5 qualification who is committed to empowering individuals and supporting meaningful daily engagement.
Key Responsibilities
- Provide person-centred support to individuals with complex needs.
- Implement and review individual support and programme plans.
- Work collaboratively within a multidisciplinary team.
- Promote independence, dignity, and inclusion for service users.
- Maintain accurate documentation and records.
- Support key worker responsibilities in line with organisational philosophy.
- Ensure health, safety, and safeguarding standards are upheld at all times.
Required Skills & Qualifications
- Full QQI Level 5 Health Care Qualification (e.g., health, community, care sector) – essential.
- Genuine passion for providing person-centred support.
- Ability to work on own initiative.
- Awareness of the Key Worker/Support Worker role and philosophy surrounding support for people with complex needs.
- Experience in programme planning and person-centred plans.
- Fluent English and excellent communication skills.
- Full, clean manual driving licence – essential.
