Key Responsibilities
- Conduct psychosocial and risk assessments for inpatients and outpatients.
- Work collaboratively with multidisciplinary teams to plan and coordinate discharges.
- Provide crisis intervention and support to patients and families in distress.
- Liaise with community and statutory services for post-discharge care continuity.
- Document interventions and maintain compliance with HSE policy and data protection standards.
- Participate in service development and quality-improvement initiatives.
- Contribute to the education and supervision of students or junior staff where required.
Required Qualifications
Essential
- A recognised qualification in Social Work approved by CORU.
- Registration with CORU at the time of application.
- Demonstrated experience in a medical, hospital, or health service setting.
- Proven communication, assessment, and case management skills.
Desirable
- Experience within acute hospital services, oncology, or palliative care.
- Familiarity with discharge planning and multidisciplinary working.
- Knowledge of HSE policies and Irish healthcare systems.
