The Health Service Executive (HSE) Dublin Midlands Health Region is seeking a Senior Medical Social Worker (Palliative Care) to join the multidisciplinary team at Tullamore Primary Care Centre, County Offaly. This permanent wholetime position offers an opportunity to contribute to a patient-centred palliative care service within a supportive, collaborative healthcare environment.
Working closely with clinical colleagues, you will provide advanced social work interventions, psychosocial assessments, and emotional support to patients and families coping with serious illness. You will play a key role in care planning, bereavement support, and promoting quality-of-life outcomes through an integrated, person-centred approach.
Key Responsibilities
- Deliver high-quality palliative and end-of-life social work services across acute, community, and primary care settings.
- Conduct comprehensive psychosocial and needs assessments for patients and families.
- Support individuals through complex emotional, financial, and practical challenges related to illness, death, and bereavement.
- Work collaboratively with multidisciplinary colleagues to develop care plans that align with patient and family goals.
- Provide advocacy and coordination of community resources and supports.
- Contribute to service development, clinical governance, and best practice initiatives.
- Participate in supervision, team meetings, and ongoing professional development.
Qualifications & Experience
- A recognised qualification in Social Work and registration with CORU (Social Workers Registration Board).
- Minimum three years’ post-qualification experience, including significant exposure to medical or palliative care social work.
- Demonstrated ability to provide complex case management and therapeutic interventions.
- Proven communication, organisational, and interprofessional collaboration skills.
- Full driver’s licence and access to appropriate transport required.

- Working Hours: Primarily 9am–5pm; flexibility required for occasional evening appointments.
According to the HSE Consolidated Salary Scales (effective 1 August 2025), the Senior Medical Social Worker salary range is approximately €68,247–€80,560 per annum, in line with Department of Health Circular 6/2025.
