The HSE West and North West is seeking an experienced nursing professional to lead our Integrated Discharge Team at Galway University Hospital, where you’ll play a crucial role in ensuring seamless transitions of care for elderly patients.
About the Role
Drive excellence in discharge planning and older persons care as Clinical Nurse Manager 2 with the Integrated Discharge Team. This permanent, whole-time position combines clinical leadership with specialized expertise in geriatric care coordination, reporting directly to the General Manager within our integrated healthcare structure. You’ll lead a multidisciplinary team focused on optimizing discharge processes for vulnerable older adults across Galway, Mayo, and Roscommon.
Key Responsibilities
- Lead integrated discharge planning for older persons across acute and community care settings, ensuring safe and timely transitions
- Manage and coordinate multidisciplinary teams including nurses, social workers, physiotherapists, and occupational therapists in discharge planning
- Develop and implement evidence-based protocols for complex discharge planning specific to older persons with multiple comorbidities
- Collaborate with community healthcare networks to establish seamless care pathways and prevent readmissions
- Conduct comprehensive assessments of older patients’ discharge needs, including home safety, care requirements, and family support systems
- Provide clinical supervision and mentorship to nursing staff within the Integrated Discharge Team
- Monitor discharge planning outcomes and implement quality improvement initiatives to enhance patient experience and safety
Required Qualifications
Essential Requirements
- Registration with NMBI as a Registered General Nurse with current PIN
- Minimum 5 years post-registration experience with at least 2 years in older persons services or discharge planning
- Proven leadership experience in nursing management or clinical coordination roles
- Comprehensive knowledge of discharge planning processes, community care services, and older persons health needs
- Strong communication and interpersonal skills for liaising with patients, families, and multidisciplinary healthcare teams
Preferred Qualifications
- Post-graduate qualification in gerontology, nursing management, or related healthcare discipline
- Experience in integrated care models and community-acute care partnerships
- Knowledge of HSE policies relating to older persons services and discharge planning
- Project management experience in healthcare quality improvement initiatives
