About the Role
As a Clinical Nurse Specialist (General) in Diabetes Integrated Care, you will work as part of the Chronic Disease Management team delivering high-quality, evidence-based diabetes services across the region. This role requires significant experience in diabetes nursing, ideally at Registered Advanced Nurse Practitioner level.
Key Responsibilities
- Provide specialist assessment, education, and management for individuals with diabetes.
- Support integrated care pathways between hospital and community services.
- Lead on health promotion, prevention, and self-management initiatives.
- Liaise with multidisciplinary teams to ensure continuity of care.
- Participate in audit, research, and service development projects.
- Deliver training and mentorship for nursing colleagues and students.
Required Qualifications
Essential
- Registered Nurse (General) with NMBI.
- Minimum five years’ post-registration experience, including substantial diabetes-related practice.
- Evidence of continued professional development in diabetes care.
Preferred
- Experience in chronic disease management or community nursing.
- Postgraduate qualification in diabetes nursing or equivalent.
- Leadership and teaching experience within a multidisciplinary environment.
