This Clinical Nurse Manager 2 role supports the Integrated Case Management Team within Primary Care Services across Cork and Kerry. The post focuses on safe discharge planning, coordinated care, and strong links between hospital and community services.
Table of Contents
You will lead nursing practice within the team and support patients with complex needs as they move between services.
Key Responsibilities
• Lead and manage nursing staff within the Integrated Case Management Team
• Support safe and timely discharge planning in Primary Care
• Work closely with hospital and community teams to improve patient flow
• Oversee clinical standards, governance, and quality of care
• Support service planning and ongoing service improvement
• Supervise, mentor, and support nursing staff
• Manage resources in line with service needs and policies
Required Skills & Qualifications
Essential
• Registration as a nurse with the Nursing and Midwifery Board of Ireland (NMBI)
• Minimum experience required at senior nursing level as per HSE criteria
• Experience in Primary Care, discharge planning, or integrated care
• Strong leadership and communication skills
Desirable
• Experience in case management or community-based services
• Postgraduate qualification relevant to the role
Salary, Benefits & Working Hours
• Salary in line with HSE Clinical Nurse Manager 2 pay scale
• Approximate range: €61,463 – €77,666 per annum (LSI may apply)
• Permanent Wholetime and Specified Purpose Wholetime contracts available
• Standard HSE terms and conditions apply
