St Columcille’s Hospital is seeking an experienced Clinical Nurse Manager 2 (GP Liaison Officer) to strengthen integration between acute hospital services and community-based General Practitioners. This leadership role is central to improving patient pathways, communication, and continuity of care across primary and secondary services.
Table of Contents
Key Responsibilities
• Lead and coordinate GP liaison services within the hospital
• Act as the primary point of contact between hospital teams and community GPs
• Facilitate safe, timely admission and discharge processes
• Support integrated care pathways and chronic disease management
• Provide clinical leadership and professional guidance to nursing staff
• Participate in service planning, audit, and quality improvement initiatives
• Ensure compliance with HSE policies, clinical governance, and patient safety standards
Required Skills & Qualifications
• Registration as a Nurse with the Nursing and Midwifery Board of Ireland (NMBI)
• Minimum required post-registration experience appropriate to CNM2 grade
• Demonstrated leadership and management experience
• Strong communication and multidisciplinary collaboration skills
• Experience in acute and/or community healthcare settings
• Evidence of continuous professional development
Salary, Benefits & Working Hours
• Salary: Clinical Nurse Manager 2 salary scale €61,463 – €77,666 (inclusive of LSI), effective from 1 August 2025
• Hours: Whole-time permanent contract
