Job Overview
The Community Access Facilitator Sligo Leitrim role offers an exciting opportunity to support individuals attending Disability Day Services across Sligo and Leitrim. Working within HSE West and North West, you will play a vital role in promoting inclusion, independence and meaningful community participation.
This position is based across Copperbeach Day Centre, Cloonamahon and Ballytivnan Training Centre, Sligo. The successful candidate will contribute to person-centred programmes that enhance life skills, community engagement and social development.
Key Responsibilities
- Facilitate access to community-based activities and inclusive programmes for service users.
- Support individuals in developing social, vocational and independent living skills.
- Implement person-centred plans in line with HSE policies and best practice standards.
- Promote positive behaviour support approaches and safeguard service users’ wellbeing.
- Liaise with multidisciplinary teams, families and community partners.
- Maintain accurate records and contribute to reporting requirements.
- Ensure compliance with HIQA standards and relevant regulatory frameworks.
Required Skills & Qualifications
- Relevant qualification in social care, disability studies, community development or a related field.
- Experience working with adults with disabilities in a day service or community setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Full clean driving licence may be required.
- Demonstrated commitment to person-centred practice and inclusion.
