Key Responsibilities
- Provide advanced specialist pharmacotherapy expertise within Galway University Hospitals.
- Conduct complex medication reviews and support optimisation of therapeutic treatment plans.

- Support medication safety initiatives and promote best practice prescribing standards.

Required Skills & Qualifications
- Be registered as a Pharmacist with the Pharmaceutical Society of Ireland (PSI).

- Proven experience in advanced clinical decision-making and complex medication review.
- Strong multidisciplinary collaboration and communication skills.
- Experience in specialist therapeutic areas including chronic disease management, polypharmacy, or high-risk medicines is desirable.
- Excellent organisational, analytical, and patient-centred care skills.
- Ability to contribute to service improvement and clinical governance initiatives.
