An exciting senior leadership opportunity has arisen for an Operational Lead – Integrated Care Programme with HSE West and North West, based in Donegal. This Grade VIII post offers experienced healthcare managers the chance to drive the delivery of integrated chronic disease management services across the Donegal Integrated Health Area.
Role Overview
The Operational Lead will provide strategic and operational leadership for the Integrated Care Programme (Chronic Disease Management) across Donegal. Working within the HSE West and North West region — which spans Donegal, Sligo, Leitrim, Roscommon, Mayo, and Galway — this permanent wholetime post plays a central role in shaping and delivering integrated, person-centred care for patients with chronic conditions.
The successful candidate will be responsible for planning, coordinating, and managing the programme in collaboration with community and acute services, ensuring alignment with national integrated care policy and regional priorities.
Key Responsibilities
- Provide senior operational leadership for the Integrated Care Programme (Chronic Disease Management) across the Donegal Integrated Health Area.
- Lead the development, planning, and implementation of integrated care pathways and service models for chronic disease.
- Work in close collaboration with acute, primary, and community care teams to ensure a coordinated, patient-centred approach to service delivery.
- Drive performance monitoring and quality improvement across the programme, using data to inform decisions and demonstrate outcomes.
- Manage and develop the programme team, fostering a culture of accountability, innovation, and continuous improvement.
- Represent the programme at regional and national forums, building partnerships with key stakeholders across the health system.
- Ensure compliance with HSE governance, reporting, and financial management requirements.
- Support the implementation of national integrated care policy within the West and North West region.
Requirements
Candidates must meet the HSE Grade VIII eligibility requirements for this post. The following qualifications and experience are expected:
- Significant senior management experience within the health service or a comparable organisation.
- Demonstrated experience in programme or service management, ideally within an integrated care or chronic disease context.
- Strong leadership, communication, and stakeholder engagement skills.
- Ability to lead change and drive service improvement in a complex, multi-disciplinary environment.
- A relevant third-level qualification in health management, healthcare, or a related discipline.
- Full eligibility to work in Ireland.

Salary & Contract
This is a permanent wholetime post, remunerated at the HSE Grade VIII scale:
€83,081 – €100,205 per annum (HSE Grade VIII scale, 7-point scale), based on the HSE Consolidated Salary Scales effective 1 February 2026.
Placement on the scale is in accordance with HSE pay policy and the candidate’s relevant service history.
Location
This post is based in Donegal, operating within the HSE West and North West health region. The Donegal Integrated Health Area serves a geographically dispersed population across one of Ireland’s largest and most rural counties, making effective programme leadership especially impactful for communities with limited access to specialist services. The Operational Lead will work across community and acute settings to ensure seamless, joined-up care for patients with chronic conditions throughout the county.
