Job Overview
The Community Liaison Nurse will provide advanced nursing leadership and coordination across community healthcare services within the HSE Dublin and North East region. The role involves working collaboratively with multidisciplinary teams, patients, families, and healthcare providers to ensure safe, effective, and seamless care delivery.

Key Responsibilities
- Provide clinical leadership and support within community healthcare services.
- Coordinate patient care pathways between acute and community services.
- Support safe discharge planning and continuity of care.
- Collaborate with multidisciplinary healthcare professionals and external agencies.
- Promote evidence-based nursing practice and quality improvement initiatives.
- Assist in service planning, evaluation, and development.
- Maintain accurate clinical documentation and reporting standards.
- Support patients, families, and carers through effective communication and advocacy.
Required Skills & Qualifications
- Registered as a General Nurse with the Nursing and Midwifery Board of Ireland (NMBI).
- Relevant post-registration nursing experience in community or integrated care settings.
- Demonstrated leadership and team management capabilities.
- Strong communication, organisational, and decision-making skills.
- Experience working within multidisciplinary healthcare environments.
- Knowledge of integrated care pathways and patient-centred care models.
- Ability to work independently while managing complex clinical caseloads.
- Full clean driving licence may be required.
