Job Overview

The Community Liaison Nurse will provide advanced nursing leadership and coordination across community healthcare services within the HSE Dublin and North East region. Key Responsibilities

  • Provide clinical leadership and support within community healthcare services.
  • Coordinate patient care pathways between acute and community services.
  • Support safe discharge planning and continuity of care.
  • Collaborate with multidisciplinary healthcare professionals and external agencies.
  • Promote evidence-based nursing practice and quality improvement initiatives.
  • Assist in service planning, evaluation, and development.
  • Maintain accurate clinical documentation and reporting standards.
  • Support patients, families, and carers through effective communication and advocacy.

Required Skills & Qualifications

  • Registered as a General Nurse with the Nursing and Midwifery Board of Ireland (NMBI).
  • Relevant post-registration nursing experience in community or integrated care settings.
  • Demonstrated leadership and team management capabilities.
  • Strong communication, organisational, and decision-making skills.
  • Experience working within multidisciplinary healthcare environments.
  • Knowledge of integrated care pathways and patient-centred care models.
  • Ability to work independently while managing complex clinical caseloads.
  • Full clean driving licence may be required.

Salary, Benefits & Working Hours
