Sligo University Hospital is seeking a highly motivated Clinical Nurse Manager 2 (Discharge Coordinator) to lead and coordinate safe, timely, and effective patient discharge processes. This senior nursing role plays a critical part in improving patient flow, reducing delays, and ensuring integrated multidisciplinary care across acute and community services.
Table of Contents
Key Responsibilities
• Lead and coordinate discharge planning across wards and services
• Promote timely, safe discharges in line with best practice and national standards
• Collaborate with multidisciplinary teams, community services, and patient flow leadership
• Monitor and analyse discharge data to identify delays and improvement opportunities
• Support bed management and escalation processes
• Provide clinical leadership, supervision, and professional guidance to nursing staff
• Contribute to quality improvement initiatives and service development
• Ensure compliance with HSE policies, procedures, and governance requirements
Required Skills & Qualifications
• Registered Nurse on the NMBI Register (General Division)
• Minimum of 5 years post-registration experience, including relevant acute care experience
• Demonstrated leadership and management capability at senior nursing level
• Strong knowledge of patient flow, discharge planning, and integrated care pathways
• Excellent communication, organisational, and problem-solving skills
• Evidence of continuing professional development
• Desirable: postgraduate qualification in nursing, management, or related discipline
Salary, Benefits & Working Hours
• Salary Scale (CNM2 – 1 Aug 2025): €61,463 – €77,666 per annum (inclusive of LSIs, subject to HSE pay policy)
