An exciting opportunity has arisen for a Clinical Nurse Manager II – Frailty Intervention Team (FIT) based in Cavan. This senior nursing role provides clinical leadership within urgent and ambulatory care services, focusing on comprehensive assessment and management of frail older adults to avoid unnecessary hospital admissions.

Key Responsibilities
• Provide clinical and professional leadership to the Frailty Intervention Team
• Coordinate and manage high-quality, patient-centred frailty services
• Lead integrated working across acute, community, and primary care settings
• Ensure evidence-based practice and compliance with HSE policies
• Manage staffing, rostering, and service delivery outcomes
• Support quality improvement, audit, and service development initiatives
• Act as a key liaison with multidisciplinary and senior management teams
Required Skills & Qualifications
• Registration with the Nursing and Midwifery Board of Ireland (NMBI)
• Minimum of 5 years post-registration experience, with relevant senior clinical experience
• Demonstrated leadership and people management skills
• Experience in frailty, older persons, urgent or ambulatory care services
• Strong communication, organisational, and decision-making skills
• Eligibility to work in Ireland

• Salary in line with Clinical Nurse Manager II (Grade 2119) HSE payscale (€61,463 – €77,666 LSI, pro rata as applicable)
