The Health Service Executive (HSE) is seeking a Candidate Advanced Nurse Practitioner (ANP) – Diabetes Integrated Care to join Community Primary Care Services across Cavan and Monaghan. This permanent, whole-time role offers an excellent opportunity to develop advanced clinical practice while delivering high-quality, patient-centred diabetes care within an integrated community setting.

This Candidate Advanced Nurse Practitioner Diabetes Cavan Monaghan role is ideal for experienced nurses working towards ANP registration who are motivated to progress into advanced practice within chronic disease management.
Key Responsibilities
• Deliver advanced clinical nursing care to patients with diabetes in community and primary care settings
• Work autonomously within agreed clinical guidelines and scope of practice
• Contribute to integrated care pathways across primary and secondary services
• Support health promotion, education, and self-management for people with diabetes
• Participate in clinical audit, research, and service evaluation
• Collaborate with multidisciplinary teams to improve patient outcomes
• Maintain accurate clinical documentation and data reporting
• Progress towards full Advanced Nurse Practitioner registration requirements
Required Skills & Qualifications
• Registration as a Nurse with the Nursing and Midwifery Board of Ireland (NMBI)
• Significant post-registration clinical experience, including diabetes care
• Working towards or eligible for Advanced Nurse Practitioner candidacy
• Evidence of postgraduate education relevant to advanced practice
• Strong clinical decision-making and leadership skills
• Excellent communication and interdisciplinary teamwork abilities
• Commitment to continuous professional development
Salary, Benefits & Working Hours
Salary Scale (1 August 2025):
€70,725 – €79,872 per annum (Advanced Nurse Practitioner – Candidate, Community / Primary Care, Grade 2276), in line with HSE consolidated pay scales
