Key Responsibilities
- Lead and coordinate discharge planning processes across multidisciplinary teams
- Ensure safe, timely, and efficient patient discharge from hospital services
- Collaborate with clinical teams to optimise patient flow and bed management
- Support implementation of hospital policies and discharge protocols
- Provide clinical leadership and guidance to nursing and support staff
- Monitor and evaluate discharge processes to improve service delivery
- Engage with patients and families to ensure clear communication and continuity of care
Required Skills & Qualifications
- Registered Nurse with active registration with the Nursing and Midwifery Board of Ireland (NMBI)
- Minimum relevant clinical experience, including leadership or management responsibilities
- Strong knowledge of patient flow and discharge planning processes
- Excellent communication, organisational, and leadership skills
- Ability to work collaboratively within multidisciplinary teams
- Experience in acute hospital settings is highly desirable
Salary, Benefits & Working Hours
Salary Scale: €62,078 – €78,443 (including long service increments)
Employment Type: Permanent Wholetime
