The HSE is seeking to recruit an Advanced Specialist Pharmacist – Pharmacotherapy for a permanent position based in Cavan. This is an excellent opportunity for an experienced clinical pharmacist to lead and develop pharmacotherapy services within a dynamic acute hospital setting.

Job Overview
The Advanced Specialist Pharmacist – Pharmacotherapy will provide expert clinical pharmacy services, with a strong focus on medication optimisation, governance, and patient safety. The post holder will play a key leadership role in advancing pharmacotherapy standards across the service and supporting best practice initiatives in line with national HSE policies.
Key Responsibilities
- Lead and deliver specialist pharmacotherapy services within the hospital setting.
- Provide expert clinical advice on complex medication management issues.
- Support and implement medicines optimisation and antimicrobial stewardship initiatives.
- Collaborate with multidisciplinary teams to improve patient safety and therapeutic outcomes.
- Develop, review and update clinical guidelines, policies and protocols.
- Participate in audit, quality improvement and risk management activities.
- Contribute to education and training of pharmacy staff and other healthcare professionals.
- Support research and service development initiatives aligned with HSE priorities.
Required Skills & Qualifications
- Registration as a Pharmacist with the Pharmaceutical Society of Ireland (PSI).
- Significant post-registration clinical pharmacy experience, ideally within an acute hospital setting.
- Demonstrated expertise in pharmacotherapy and medicines optimisation.
- Strong leadership, organisational and communication skills.
- Experience in clinical audit, governance and quality improvement.
- Ability to work effectively as part of a multidisciplinary healthcare team.
- Commitment to continuing professional development and service excellence.

- Salary aligned with HSE consolidated pay scales for Pharmacist – Advanced Specialist (Grade 3250), in accordance with February 2026 public sector pay adjustments.
