Job Overview
Cork University Hospital is seeking an experienced Clinical Nurse Manager 2 – Discharge Coordinator to lead and coordinate safe, timely patient discharge planning across services. This is a key leadership role supporting patient flow, multidisciplinary collaboration, and high-quality clinical care within a major acute teaching hospital.
The successful candidate will work within HSE South West and play a critical role in improving patient outcomes and ensuring efficient transitions of care.
Key Responsibilities
- Lead and coordinate hospital discharge planning processes across assigned services.
- Collaborate with multidisciplinary teams to ensure safe and timely patient transitions.
- Support patient flow initiatives and optimise bed management strategies.
- Ensure compliance with HSE policies, procedures, and clinical governance standards.
- Provide clinical leadership and supervision to nursing staff.
- Engage with community services to facilitate continuity of care.
- Monitor and evaluate discharge processes to identify improvement opportunities.
Required Skills & Qualifications
- Registered General Nurse with active registration on the NMBI register.
- Minimum experience requirements in line with Clinical Nurse Manager 2 grade standards.
- Demonstrated leadership and management capability in an acute healthcare setting.
- Strong knowledge of discharge planning, patient flow, and multidisciplinary coordination.
- Excellent communication, organisational, and decision-making skills.
- Commitment to professional development and quality improvement.
Salary, Benefits & Working Hours
Salary is aligned with the Clinical Nurse Manager 2 (General) Grade 2119 HSE pay scale.
As per the HSE Consolidated Salary Scales effective 1 August 2025, the scale ranges from:
€61,463 – €77,666 (LSI applicable)
