About the Role
As a Community Rehabilitation Assistant, you will play a key role in supporting adults living with an acquired brain injury to rebuild independence, confidence, and meaningful participation in their own communities. Working as part of an interdisciplinary, person-centred team, you will deliver one-to-one rehabilitation supports in people’s homes and local environments, helping them work toward goals that matter to them – whether that’s daily living skills, social connection, cognitive strategies, or community engagement. This role is dynamic and community-based, offering variety, meaningful relationships, and the opportunity to make a tangible difference in someone’s everyday life.
The contract available is Specified Purpose, Full-Time, 39 hours.
What You'll Be Doing:
- Build respectful, trusting relationships and offer consistent support
- Help create and guide personalised rehabilitation plans
- Encourage independence through daily living skills, personal care, and goal-focused activities
- Identify additional support needs for the person served
- Encourage family, personal and neighbourhood relationships and interactions
- Feeding back to the clinical and management team throughout the rehabilitation journey
- Play a key role in case coordination for supporting consistency in rehabilitation review meetings
Requirements
Must Haves:
- Relevant Health or Social Care qualification to a QQI Level 5 or higher
- Full driving licence
- Person-centred values
- Strong communication skills - be able to engage compassionately, clearly and confidently in spoken and written English
- Confident using basic IT systems to stay connected and organised
Great to Have:
- Experience in Residential or Community settings
- Background in Disability or Illness support
