About the Role
Peamount Healthcare is seeking a Discharge Planner (CNM2/Senior HSCP) for a permanent, full-time position in Newcastle, Co. Dublin. The role involves supporting all discharge pathways, managing beds, and ensuring safe and proactive planning of discharges for patients with complex needs. The successful candidate will work in consultation with the Patient Flow and Operations Lead, senior clinicians, and acute partners. The role requires a registered nurse or health and social care professional (HSCP) with relevant experience and excellent communication skills. The salary for this position is €62,699–€79,227 per year.
Requirements
Eligibility Criteria:
- Be registered in the General Division of the active Register held by Nursing and Midwifery Board of Ireland or be entitled to be so registered/CORU registered where a HSCP
- Have 5 years relevant post registration experience and a minimum of 2 years’ experience in the acute hospital settings
- Have the clinical, managerial and administrative capacity to properly discharge the functions of the role
- Have excellent communication and interpersonal skills
- IT competency in Microsoft 365
- Demonstrate evidence of continuing learning and professional development
- Be able to demonstrate managerial abilities
