Purpose of the Post:

• To support the regional development and implementation of rights-based, person-centered and community-based Disability Home Support arrangements, that promote dignity, autonomy, will and preference, inclusion, safeguarding, quality and accountability.
• To support the regional readiness, implementation and the on-going administration/ co-ordination of the new Disability Home Support Authorisation Scheme, in association with disability teams within the IHAs and CHAs
• To support the Authorisation Scheme’s data collection, analysis, reporting, process and administrative requirements, to improve consistency, quality, and equity of access, integration and outcomes for people with disabilities.
• To support the HSE Home Support Reform Programme and the roll out of new Home Support service development initiatives and/or new requirements across the region, working in an integrated manner with regional implementation groups, local disability teams and colleagues in Older Persons and Mental Health Home Support services.

Community Healthcare Services are delivered through the HSE and its funded agencies to people in local communities, as close as possible to people’s homes. Community Healthcare Services focus on keeping you well so that you can continue to live at home or close to home through our health promotion, disease screening, diagnosis, treatment, and rehabilitation programs. You can refer yourself to most Community Healthcare Services or through your GP, public health nurse, community mental health team, etc.

Community Healthcare Services cover a broad range of services that are delivered outside of the acute hospital setting and include:
