Who We Are
Caredoc was founded in 1999 as the first GP Co-Operative in Ireland. Caredoc now provides many healthcare services across the Southeast, East coast, Midlands, and Northwest of Ireland. Caredoc works in partnership with GP's, HSE and the Department of Health and has its headquarters at St Dympna’s Hospital, Athy Road, Carlow.
The Caredoc Out of Hours service provides high quality, easily accessible GP care for urgent medical problems outside of normal surgery hours. Patients contact the service by phoning a low call number and their details are logged onto an expert system. Triage nurses provide remote assessment and determine the level of care that is appropriate. Patients are seen by experienced doctors local to where they live and in the case of elderly and infirm patients a driver facilitates the doctor to carry out domiciliary visits.
Caredoc also provides Community Intervention Team (CIT) services in the Southeast and East coast areas. Nurses, Physiotherapists, and Occupational therapists work together to provide acute care to patients discharged home early from hospital and help to support patients in the avoidance of hospital admission. The CIT service works in close collaboration with GPs, acute hospitals, public health and community care. Teams are based in Carlow/Kilkenny, South Tipperary, Waterford, Wexford and Wicklow.
The Caredoc SMILE service is an innovative way for patients with chronic disease to proactively self-manage their health and well-being using a digital platform and supported by nurses. Patients with two or more conditions including diabetes, COPD, chronic heart failure, and heart disease use digital devices to monitor their health and wellbeing. This virtual case management of multi-morbid patients is an essential element of the Integrated Care Programme for the prevention and management of chronic disease clinical model of care and hence is an essential part of the Enhanced Community Cared programme. SMILE is rolled out across the Southeast area and patients are referred by GP’s, ECC HUB Nurse specialists and therapists, hospital and integrated consultants. Caredoc - Out of Hours Doctor Service
