About the Role
HSE Dublin and South East has been formed as part of the Slaintecare programme. The health region is responsible for the delivery of acute and community healthcare across South Dublin, Wicklow, Wexford, Waterford, Carlow, Kilkenny and South Tipperary.
There is currently one permanent part-time post available in the RHA HSE Dublin & South. The commitment for this post will be for 30 hours per week to work within South Tipperary Chronic Disease Community Specialist Service and aligned hospital Tipperary University Hospital.
In line with the Model of Care, 80% of the CNS role will involve working with MDTs including General Practitioners (GP’s) in the Chronic Disease Community Specialist Service, and 20% of CNS role will involve working with the IC Cardiology Consultant in the Chronic Disease Community Specialist Service and/or in Secondary Care OPD. There will be a strong focus on service integration and team-working.
A panel may be formed as a result of this campaign for Clinical Nurse Specialist Cardiology - Integrated Care from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled for HSE Dublin and South East.
