About the Role

Integrated Healthcare Area Sligo Leitrim is responsible for the delivery of Primary and Community-based Services to meet the needs of local communities.

Community healthcare services are a broad range of services, which, are provided outside of the acute hospital system and include Primary Care, Social Care, Mental Health, and Health and Wellbeing Services. These services are delivered through the HSE and its funded agencies to people in local communities, as close as possible to people’s homes.

Sligo/Leitrim Disability Services provides a wide range of health and personal social services to adults with disabilities. Services delivered through the model of health and social care include the following:

• Residential placements
• Day Services
• Respite bed nights

To assist in the provision of health and social care for adults with an intellectual disability. The Care Assistant provides direct care in line with the individual’s Person Centred Plan.

Salary: €34,881–€48,434 per year

Location of Posts: Sligo/Leitrim Disability Services operates a model of community living and we currently have over 30 community group homes in areas throughout Sligo and Leitrim. This is expected to increase in line with the National Policy for Decongregation and Community Integration.
