SLPC2630 Physiotherapist, Clinical Specialist in Urology (Pelvic Health)
Integrated Healthcare Area Sligo Leitrim is responsible for the delivery of Primary and Community-based Services to meet the needs of local communities. Community healthcare services are a broad range of services, which, are provided outside of the acute hospital system and include Primary Care, Social Care, Mental Health, and Health and Wellbeing Services. These services are delivered through the HSE and its funded agencies to people in local communities, as close as possible to people’s homes.
This role has been created as part of the development of the Continence pathway. The Continence pathway is one of a number of modernised care pathways (MCPs) developed by the National Clinical Programme in Surgery (NCPS) and the HSE as a core component of the transformation of the delivery of scheduled care services.
There is currently 1 whole time permanent Clinical Specialist Physiotherapist positons available in the Urology service to support the National Urology Model of Care 2019. The posts will be located in IHA Sligo/Leitrim/South Donegal/West Cavan.
The aim of these posts is that those referred to the Urology Continence service will receive the right care, at the right time, in the right place in keeping with Slaintecare principles. To provide a first point of contact community Urology Service, encompassing the physiotherapy Advanced Assessment, management and treatment of patients experiencing urinary or bowel incontinence, in conjunction with other member of the MDT, to provide a continuum of care. This post will facilitate specialist interdisciplinary assessment in order to reduce waiting times and improve care delivered to patients within the Urology Continence service. To work with Urology Consultants, Continence Nurse Advisors and other relevant health care professionals in undertaking the triage of ambulatory urology related referrals (efficient and effective management of the waiting lists) to improve the pathway of care within this group, thereby enhancing efficient access to the related services as required. To act as a lead in the development of the Physiotherapy service provided to clients under the National Urology Model of Care 2019. Taking a lead in implementing the improved service model for Urology across primary care in CHO West and as an in-reach secondary care service within the Saolta hospital group. The post holders will be required to collect and analyse data to evaluate effectiveness of their input. They will be required to produce and present reports intermittently as required.
