HSE HR Forms Explained: Every Form & When You Need It

Almost everything you do as an HSE employee — taking leave, a career break, going on reduced hours, claiming an allowance — starts with an HR form with a code like HR105 or HR108j. This guide lists the forms you'll actually use, what each is for, and where the entitlement behind it is explained. All forms are downloaded from the official HSE HR forms page and submitted through your line manager / local HR.

  • HR104 — change of personal details (name, address, bank).
  • HR3 — employee transfer; HR102 — change to terms and conditions.
  • HR101 — employee set-up (new starters); HR103 — temporary appointments.
  • Travel & subsistence claim and BT1 (foreign travel approval).

Download the current version from the official HSE HR forms page (forms are updated — don't reuse an old saved copy), complete it, and send it through your line manager to your local HR/SAP HR department. What is HSE form HR105?
HR105 is the HSE career break application form. The related HR115 is for the Shorter Working Year scheme. Our career break guide explains eligibility and how both schemes work.
What form do I need for HSE parental leave?
HR108j for Parental Leave (the 26 weeks unpaid per child) and HR108t for Parent's Leave (the 9 weeks paid). They're separate schemes with separate forms.
What is the HR108 form series?
HR108 with a letter suffix covers most HSE leave types — e.g. HR108i maternity, HR108k paternity, HR108c carer's leave, HR108d bereavement, HR108f force majeure, HR108n marriage leave, HR108o special unpaid leave.
What form starts Temporary Rehabilitation Remuneration (TRR)?
HR114b is the TRR form and HR118 covers the TRR top-up payment — though in practice HR usually initiates TRR when your paid sick leave is exhausted. HR113 is the separate Critical Illness Protocol application.
